Provide behavioral health services including specialized assessment, coaching and prevention services for individuals and families in the area of substance abuse, and mental health using a combination of Western and traditional methods.
2.
Provide care coordination for patients, acting as a point of contact for patients needing information about care coordination and appointments.
Make referrals to other service providers as appropriate and assist with required paperwork.
Make patient call reminders for appointments and assess barriers for missed appointments.
3.
Provide client advocacy, referrals, client monitoring, case management, and follow-up to patients.
4.
Co-facilitate individual and group sessions with the counselor.
Organize groups and other activities for patients to have opportunities to engage in alcohol / drug free social activities that promote good health and wellness skills.
5.
Seek to develop traditional counseling skills and service for individuals and families in the areas of mental health, substance abuse, and prevention specific to the needs of patients residing in Indian Country.
Position will receive training from White Bison for culturally rooted interventions and strategies.
6.
Participate in weekly MAT and provider meetings and collaborate with counselors, PRS staff, medical staff, Program Director, and support staff.
7.
Provide crisis intervention and / or coordinate post inpatient services, as needed.
8.
Perform UA's.
9.
Complete GPRA and other screeners with clients to gather baseline data, individual patient progress, and program outcomes.
Prepare and submit required reports and activity sheets in accordance with grant protocols.
Maintain client charts as required.
10.
Work towards achieving grant objectives through activities and all other duties as assigned by supervisor.
11.
Complete other tasks as requested by Program Director.
12.
Assist with transportation services as needed.
13.
Complete all paperwork in a timely manner.
Qualifications:
1.
Must have a high-school diploma or GED.
However, Bachelor's degree in Social Work, Psychology, or other behavioral health related field is preferred.
2.
Must have a valid driver's license.
3.
Upon hire, must be willing to complete additional training for Peer Recovery Support Services Certification and Traditional focused interventions.
4.
Knowledgeable about community, state, and federal resources; general knowledge of co-occurring, psychosocial, cognitive, and / or developmental disorders in adults.
5.
Previous experience working with adult patients in substance abuse and / or trauma focused treatment.
6.
Candidate enjoys working in a collaborative teamwork environment.
Applicants must be able to pass a pre-employment drug screening and background check.
